@kjamytruk Turns out the lady who dosed me on mar29 gave me my form back in error, so it was never filed. MB Health suggested I return to RBC to provide the form. Did that, record corrected immediately. Glad I didn't throw it out... the record was actually supposed to go to data entry!

@kjamytruk My 1st dose (Mar 29) was not reg'd, 2nd dose was. Took me 5 weeks of forms, 14-day waits, and repeat phone calls to figure out the MB Health angle. Got on with a supervisor, she said the paper "record" I had from March (RBC Centre supersite) should not be in my hands...
