Anybody else’s broker told them this after asking them why they haven’t informed me about needing to file proof of interest? I also asked how can I get my certificates off them to show interest, I shared with them the link to the court doc, I showed them the proof of interest form and this was what they replied with in the picture below!!

Why is everything so fucking awkward in this market. Nothing is ever straight forward!! Sick to death with it all!!

The form is fairly self explanatory - make sure the name of the owner of the equity interest reflects the same name on your stock certificate, make sure you triple check the stock certificate number, The "Amount of Membership Interest" are your shares of stock (the quantity), "Date Equity Interest was Acquired" is the first date you purchased the security - if you bought more than once, you can put date ranges in (between October 1, 2021 and December 9, 2022 - just an example) Some people like to list the exact dates and purchases, that could be a better practice to do, just to cover all bases, most of you will check the box that you own the equity securities in the debtor (Type of Equity Interest) you should supply your brokerage account statements showing the dates on which you acquired the equity securities and if you are the owner, check the box "I am the Creditor". This is just a guide, I suggest you speak to a bankruptcy attorney that does this every day for further detail and instruction.
