Pitting edema occurs when excess fluid accumulates in the interstitial tissues. Applying pressure with a finger temporarily displaces this fluid, leaving a visible indentation that slowly refills. It is a clinical sign, not a diagnosis, and always warrants evaluation for the underlying cause.

In heart failure, reduced cardiac output activates the sympathetic nervous system and the renin angiotensin aldosterone system (RAAS), leading to sodium and water retention by the kidneys. At the same time, elevated venous hydrostatic pressure increases the movement of fluid from the capillaries into the surrounding tissues. Gravity makes this fluid collect predominantly in the ankles and lower legs, producing bilateral pitting edema. As heart failure progresses, patients may also develop exertional dyspnea, orthopnea, paroxysmal nocturnal dyspnea, fatigue, elevated jugular venous pressure, pulmonary crackles, and rapid weight gain due to fluid retention.

However, heart failure is only one cause of pitting edema. Chronic kidney disease or nephrotic syndrome can lead to sodium retention and hypoalbuminemia, reducing plasma oncotic pressure and allowing fluid to leak into the tissues. Chronic liver disease causes reduced albumin synthesis and portal hypertension, both of which contribute to edema. Chronic venous insufficiency increases venous pressure in the lower limbs, while deep vein thrombosis usually causes unilateral painful swelling. Lymphedema, in contrast, is classically non pitting in its later stages because protein rich fluid stimulates fibrosis of the surrounding tissues. Certain medications, including dihydropyridine calcium channel blockers, NSAIDs, corticosteroids, thiazolidinediones, and some hormonal therapies, can also cause peripheral edema.
