In our study of 224 node-positive breast cancer patients treated with neoadjuvant chemotherapy, we found that axillary and locoregional recurrences were rare (3.4%) in a selected group of yN0 patients who underwent SLNB and/or ALND. Additionally, SLNB with fewer than two positive nodes rarely resulted in a significantly different yield of positive non-SLNs or impacted short-term oncologic outcomes compared to ALND. These findings suggest that de-escalation of axillary surgery to SLNB alone may be a safe option in this patient population.
