1/ Person walks into pharmacy and asks for a BP check. Nothing too exciting, but pulse flagged as irregular on the machine. No CV history or antihypertensives. Discussed if they had ever been told about irregular pulse, murmur, arrhythmia or atrial fibrillation. “You mean AF?!”

A few days later the RN that admitted the patient after my call comes into the pharmacy and thanks me for sending the patient in.
Multiple PE’s and required transfer to a larger regional hospital for mgmt.
