Interested to know other community Trust processes around sending bereavement surveys following EOLC provided by DN teams, how data is collected, how often this is analysed etc…

Q: How soon can a new service in the community be created to help prevent unnecessary admissions, accelerate discharges & keep people safe at home?
A: The 24/7 District Nursing service does this now & coordinates health & social care. Invest & strengthen the existing DN services.
