Hear hear. We often (though not enough) talk about provider workflow as a digital health priority, but patient (and caregiver) workflow innovation would be even more impactful if we could actually get to a viable patient-mediated platform.
While we talk of a #platform for #diabetes care or #kidneydisease care or others, this is a problem for #patients who often don’t have just one disease. IMO this is like the multiple pt portal problem. A real platform would unify various care management tools. #MITPlatforms
