Two possibilities. One is negligence and the other is something I would have to put on my tinfoil hat to say.

Here’s the reality: Five grams of pentobarbital stops breathing in a minute or two. When someone is still snoring 40 minutes later, the drug didn't get in. So either the IV slipped or the vein blew and it pooled in the arm, the line flowed too slowly, or the compounded drug was weak.

Tennessee has now had two failed executions in one year. In the last attempted execution they couldn't get an IV into Tony Carruthers either.

I’m old school, I use a mop and sink. I use my hands to rinse and squeeze all the water out, mop the floor and start again.
Splash a little fabuloso on the floor and go. I can’t do the swifter and things like that. I don’t feel like the floor is clean enough with those.
