This gastric body biopsy shows severe destructive gastropathy - the parietal cells are gone! It is too inflamed for autoimmune gastritis/pernicious anemia. Apoptotic bodies, usually scarce in the stomach, are indicated. This was a result of pembrolizumab immunotherapy.

This week, I learned not only that gangliocytic paragangliomas were reclassified by the WHO as composite gangliocytoma/neuroma and neuroendocrine tumor (CoGNET) - thank you Rhonda Yantiss - but also that they can express CD117/KIT to result in wrong diagnoses on small biopsies.
These small intestinal biopsies from a patient with markedly elevated serum IgM from a splenic plasmacytoid extranodal marginal zone (mucosa-associated lymphoid tissue) lymphoma show dramatic edema and IgM deposition that resulted in malabsorption. There is no lymphoma.
