"I've kept the paper in my wallet for 10 years, and now it's lost," she told us that day in the hospital, as her partner for days attempted to retrieve the certificate about the cochlear implant (external unit in the photo) she got some 20 years ago.
Simona was born with a profound sensorineural hearing loss, so the device provided her with sound perception. This fundamentally changed her life and helped her live an increasingly independent life.
Unexpectedly, the device turned out to be the main impediment for doctors to identify metastases in her brain using MRI. Earlier discovery of how cancer had progressed could have drastically impacted the outcome.
More precisely, it was the lacking documentation on her cochlear implant that discouraged them from MRI scanning her head - looking into the hospital's database and her medical record, they were not able to establish with confidence whether or not the device she got could be hazardous with MRI due to metal parts.
Neither were we able to retrieve the certificate of the upgraded cochlear implant following the surgery about 10 years ago, to cross-check with records kept at the hospital.
Sadly, Simona unexpectedly lost consciousness and she was rushed into a surgery room a couple of days later. The surgeon told us how surprisingly fast the cancer had progressed, entangled even around the cochlear implant, and that her chances for survival were very small.
The surgery proved to be immensely difficult, hindered by the poor knowledge of the state of cancer - the state that could've been clearly established, had MRI been performed earlier. She would cling to slim chances to live, but passed away a couple of weeks after the critical surgery at @ukclj. The doctors and the medical staff did an incredible job fighting to preserve her life and later her dignity during the coma.
Today marks one year since a radiology exam (CT) showed a brain cyst in Simona's head. What initially seemed to be a rather manageable condition, which we believed was properly tackled during the first surgery, was in fact a much more serious state in which a malignant cancer commenced to spread aggressively within her brain.
Having lost Simona to such a treacherous disease, there are so many "what if-s", yet there is one of them which doesn't cease to stand out - would proper record-keeping about the cochlear implant have allowed doctors to go on and perform the needed tests (including MRI) to establish the actual state of cancer spread, and would an earlier and more thorough surgery have given her a chance to live?
We will never know.
This can however serve as an appeal to those in charge of maintaining patient records, to do everything in their power to ensure that medical records (including the medical device certificates) are properly maintained and verifiable.
In this day and age, this particular "what if" - the doctors had the right information at the crucial stage of treatment - must be avoided by all means available.
Let my sisterโs story be a catalyst for change, inspiring those in charge of patient records to implement robust systems in the future. In doing so, we honor her memory and strive to create a healthcare environment where every patient receives the care they deserve, guided by the right information at every critical juncture.

For many years now the Swiss @RailService SBB has been leveraging @origin_trail Decentralized Knowledge Graph to track their assets and using Knowledge Assets and @gs1 standards.
One of the applications they are using this knowledge for is predictive maintenance. SBB performs train track welding monitoring, recording every single welding event in their entire railway network in Knowledge assets to successfully predict where weldings have the highest risk of breaking, so they can service them in advance. Welding events are recorded as @gs1 EPCIS events, then connected in the @origin_trail DKG, enabling AI-powered neuro-symbolic queries.
