No Dr, surgeon or healthcare provider in America, if they’re being completely honest with you, can say that they’ve never made a mistake (tired or otherwise) while caring for their patients. We are only human after all.
Now, A serious mistake? A mistake that resulted in patient harm? No.

That’s not me trying to say how amazing I am. That is the result of a combination of dedication, skill, great training, not cutting corners, and robust safety systems that have been put in place over decades of physicians making mistakes and learning from them.

If I had the option to only work 40 hrs a week for less pay I would gladly take it, but that is simply not how hospital based healthcare is set up these days.

People don’t only get sick between the hours of 9-5. Hospitals don’t stop providing care when the sun goes down. Nearly all hospitals are required to have surgical, anesthesia, and Ob staff immediately available or in-house, 24/7 365.

Congress has been pillaging Medicare and Medicaid for decades while simultaneously supporting bills that make their donors richer and Americans less healthy. This has led to healthcare demands and costs increasing while available funds are decreasing. How does Congress/CMS solve it? By voting year after year to decrease hospital and physician reimbursements. The Big Insurance companies all base their own reimbursements on what CMS (Center for Medicare/Medicaid Services) does. That leads to increasing pressure on hospitals to squeeze out more “productivity” from fewer resources.

As an example, in 2023 CMS decided to cut physician reimbursement by 3.37%. Then in November last year they voted to cut physician reimbursement again by 2.87%. At the same time they voted to cut reimbursement for Anesthesia services by 2.20%. Even worse, back in 2017/2018 CMS arbitrarily decided that GI procedures like colonoscopies did not require an actual Anesthesia provider to administer Anesthesia (ie a regular nurse was sufficient) except in very sick patients and most insurance companies followed suit!! That’s completely insane and only shows how little these bureaucrats understand about just how dangerous Anesthesia really is, especially in GI cases!

What those cuts result in, is hospitals and physician groups being forced to do more procedures with fewer staff, to increasingly rely on mid-level providers (NP’s, PA’s, CRNA’s, etc) with physician “oversight”, and for those physicians who remain to work longer hours.

Covid only made things worse. A lot of US physicians were over age 60 when COVID hit. Many older docs died from it, and even more retired rather than take the risk or the jab. Meanwhile for years Fewer and fewer grads have been choosing to become physicians because the training is long and arduous, and because the hours/demands even after you graduate are so taxing. Work-life balance is extremely challenging, school debt is astronomical and income keeps going down every year. Instead they’re opting out of the medical field altogether or to become mid-level providers instead. All of these things further increase the demands on those of us MD’s who are left!

So, is it more likely I’ll make a mistake when I’m tired and overworked?? Yes of course it is. But don’t think for one second that I am doing this for the money or that I would ever sacrifice patient safety in the name of profit. I have an IQ of 186 and had a 4.5 GPA in undergrad. I have a bachelors, two masters degrees and a doctorate. I could’ve easily gone into business, stock trading, or become a CEO. If I had, I’d be making many times what I earn right now with fewer headaches, less debt and a far better lifestyle!!! I do this because I love what I do, because I genuinely care about the people who entrust their lives to me every single day, and I work the hours I work because I’d I didn’t there wouldn’t be anyone to take my place and people would die.
