This is mostly ALL youth sports because of unprepared coaches lacking the knowledge of how to play the game and they only coach the RESULT, not the PROCESS.

IMO:

1. Runner would've been safe. No advancement. No error. All runs earned.

2. Throw is an error. Runners advanced. Proceeding runs are earned bc of following hits allowed.
