In a world-first medical milestone, surgeons have successfully performed the first-ever HIV-positive-to-HIV-positive lung transplant.

56-year-old Bertrand Nelson of New Jersey became the first person in the world to receive a double lung transplant from an HIV-positive donor. The groundbreaking procedure, performed on March 21, 2026, at NYU Langone Health, also included a simultaneous liver transplant. Nelson had been suffering from end-stage organ failure due to advanced sarcoidosis, a condition that severely damaged his lungs and later affected his liver.

While kidney, liver, and heart transplants from HIV-positive donors to HIV-positive recipients have been conducted under the U.S. HIV Organ Policy Equity (HOPE) Act, lungs had previously remained off-limits due to higher infection risks and surgical complexities. This successful operation significantly expands the donor pool for the thousands of people living with HIV who need life-saving transplants.

Nelson is now recovering at home, off oxygen for the first time in years, and is actively sharing his story to reduce stigma around HIV. He encourages others living with the virus to register as organ donors and wants to send a powerful message to HIV-positive patients on transplant waiting lists: “You are worthy.”
