One of the five great Shinigami clans (like Byakuya and Yoruichi's), the Tsunayoshiro clan, hunted Fullbringers to collect their fragments.

Tokinada Tsunayoshiro was responsible in this case and tricked Ginjo and Ukitake to cause this entire tragedy.

(Tokinada was also the same Shinigami who killed Tousen's friend, and he was responsible for Tousen's downfall into evil.)

I see many authors who put their work above their health, end up getting sick, and have to go on hiatus to recover.

If you notice a physical issue, go to the doctor to treat it before it becomes serious—don't wait until the situation is already critical.
