⚡ Can impedance sensing provide real-time insight into clot composition during mechanical thrombectomy?
In this first-in-human JNIS study, an impedance-sensing guidewire demonstrated high accuracy in distinguishing red blood cell–rich from platelet-rich clot components during mechanical thrombectomy.
Impedance sensing also identified clot boundaries in situ, with no device-related intracranial vessel perforations or dissections reported.

💡 What is the current evidence on coil-only middle meningeal artery embolization (MMAE) for chronic subdural hematoma (cSDH)?
In this JNIS scoping review, authors analyzed 9 studies including 367 patients and 459 embolization procedures evaluating coil-only MMAE for cSDH.
Recurrence rates ranged from 0–17.6%, reintervention rates from 0–15.4%, and complication rates from 0–5%. Techniques varied from proximal MMA trunk occlusion to more distal branch-level coiling, with potential advantages of coil-only MMAE in anatomically challenging cases.

🧊 This in vitro study demonstrates that catheter insulation, flow rate, and proximal blood flow control are key determinants of intra-arterial brain cooling efficiency. 🧵 Can endovascular embolization serve as a standalone treatment for select intracranial meningiomas?
In this multicenter study, authors evaluated 41 intracranial meningiomas treated with standalone embolization across four centers. Tumors demonstrated progressive regression over time, with median volume reductions of 28.6% at 1 month, 57.1% at 6 months, and 81.7% at 12 months.
An early devascularization index predicted greater subsequent tumor shrinkage. Complications occurred in 4 of 41 lesions, with one resulting in a mild clinical decline. No patient required subsequent surgery or radiotherapy.

#SNIS2026 Day 3 Lecture Spotlight: Dr. David Fiorella shared encouraging early results on the use of the TRUFILL™ n-BCA Liquid Embolic System for patients with refractory migraine. Although the trial is still ongoing, the benefits of embolization appear to be durable, with improvements maintained out to 6 months. Importantly, he emphasized that while these single-arm results are promising, a randomized controlled trial will be essential to confirm efficacy.
