During my paramedic career, if I had ever taken 2 hours to transport a trauma patient <4 miles, I would have been fired and had my license taken.

Most trauma emergencies are surgical emergencies that are fixed in the OR. Only a few reasons exist to delay transport, since most interventions can be done en route to the hospital.
Most medical emergencies benefit greatly from on-scene treatment and stabilization before transport.
