The workflow in practice (Claude Code): /opsx:propose → write change + specs + design /review-change → spec-reviewer audit /opsx:apply → implement task by task /git-commit → commit to spec + Jira /ship → archive + merge: Spec → Task → Commit → PR.

Two planes, never confused: Management plane (Jira) → what work exists Governance plane (OpenSpec) → how the system must behave A Jira ticket, no matter how detailed, does NOT authorize code. A reviewed OpenSpec change does. This inversion is everything.

The principle: Spec → Plan → Code Code is the LAST artifact produced, never the first. Before any implementation, there's a change document: → proposal (why) → delta specs (what the system will do) → design (how) → task list (steps) Only then does the agent write code.

The problem: Claude Code is an excellent executor and a terrible architect. Give it a task → it fills in design decisions on its own → those decisions end up in production, untraceable, unreviewed. The fix isn't better prompts. It's structure.
