In her role as Locality Coordinator, Sally works in community nursing and has strong links with secondary and social care.
She works as a social prescriber to assess, decision make, problem solve and signpost to enable patients to remain safely at home.

Speech and Language Therapist, Amelia, works with children in primary and secondary school who have additional learning needs.
Her role is to assess language development and speech making care plans which could include 1:1 therapy or school support.

Angie works in the community, assessing and recommending major environmental adaptations for adults with physical disabilities, such as stair lifts, ramps and wet rooms.
She's proud to be an AHP because her work makes a difference to patients every day.

Alexandra works at the Neuro Rehabilitation Centre and Ward 3 at Goole hospital, with patients who have progressive neurological conditions.
She says: "I give patients a voice and advocate for them, enabling them to live their lives to the fullest.”
