After that, a Collagen plug was placed to cover the site and sutured with Glycolon . Finally an ovate Pontic supported by essix retainer was delivered by @draljameel to start shaping the site immediately.

🇺🇸Part1: A long story behind this tooth!
The pt came to see me after 2 failed RCT and another failed apicoectomy.
The Endodontist referred her after noticing a crack at the apical part of the tooth via microscope!
Since then,

the pt has persistent sinus tract, and episodes of swelling and suppuration.
The plan was explained to extract and clean the defect through the socket then seal the fenestration with a collagen membrane and graft the site with Cortico-Cancellous Allograft.

Debrided the socket, grafted with @maxxeus FDBA cortical bone, @biohorizonsuniversity collagen mem-lok membrane, and topped it off with @salvin amnion chorion membrane. 4.0 chromic gut sutures to close.

An interesting case of possible extensive root resorption at the distal root of all first molars that increased with time (check the panoramic x-ray)

16 years old patient presented with severe pain at upper first molars, deep distal PD and furcation involvement.
