🫀How should we diagnose & manage #HCM?
This #EJHF expert consensus statement addresses the gray zones clinicians face in daily practice including: 📍Cardiac myosin inhibitors (#mavacamten, #aficamten) are now central to oHCM care 📍 SCD risk uses a hybrid model: ESC 5-year calculator as the starting point (ICD recommended if ≥6%, not if <4%), with LGE >15%, apical aneurysm or sarcomeric variants tipping borderline (4 to 6%) cases toward implantation 📍Septal reduction therapy works best at high-volume centres 📍Tailored management for pregnancy, #AFib, hypertension & paediatric HCM 📍Future directions: gene therapy, AI-ECG & novel biomarkers

We review current evidence surrounding risk stratification, lesion-specific strategies, anticoagulation considerations, and multidisciplinary cardio-obstetric care in this high-risk population.
Thankful to all my co-authors, especially my mentor @DesaiMilindY for their support!
In 3,546 patients with obstructive HCM undergoing surgical myectomy at the Cleveland Clinic, earlier referral before advanced symptom progression was associated with more favorable long-term outcomes.
🫀Happy to share our work on Long-term outcomes of oHCM undergoing surgical myectomy: Impact of symptom severity at presentation
This study represents the largest published single-center surgical myectomy experience to date.

Landmark #whyCMR paper on HCM
From @JAMA_current
Among patients with #hypertrophic#cardiomyopathy followed for nearly 7 years, key predictors of adverse outcomes included
1. late gadolinium enhancement (LGE) percentage on #whyCMR,
2. left ventricular (LV) mass index,
3. LV end-systolic volume index,
4. history of heart failure, and
5. log(N-terminal pro–B-type natriuretic peptide [NT-proBNP]).
LGE ≥9% of LV mass was associated with substantially increased risk for the composite of hypertrophic cardiomyopathy–related deaths, ventricular arrhythmias, and transplant or LV assist device implant.
These findings provide prospective evidence for integrating #whyCMR and blood biomarkers in risk assessment for hypertrophic cardiomyopathy. #HCM

Congratulations Dr. @ChrisKramerMD and others from HCM registry on this terrific paper.
Over the weekend at the @escardio Heart Failure Congress, @DesaiMilindY presented data that provided a closer look at our optimized immune suppression and monitoring protocol in the MyPEAK™-1 Phase 1b/2 Clinical Trial of TN-201. Learn more: https://t.co/vPGUzoHQ44

Can #CCTA stratify heart failure risk through radiomic phenotyping of epicardial adiposity?
We derived & externally validated a deep radiomic signature of epicardial fat in >70,000 adults.
