I’ve worked a couple places where vanc and Dapto are first line for MRSA osteo, endocarditis, bacteremia, PJI etc. convincing folks (including myself) otherwise is a challenge.

Maybeee Linezolid but it makes me nervous for an extended course. TID Vanc is sometimes needed even when using AUC/MIC. Even high dose BID Vanc is a pain for pts at home. Long infusion times.
