PEA and luteolin help restore the sense of smell after COVID.

In a trial, over half of the patients regained a normal sense of smell, compared to only 10% in the placebo group.

Anosmia, aka smell blindness, is common after COVID and can improve with olfactory training. Certain substances can improve or accelerate recovery. These include vitamin A and zinc.

In this study, patients with COVID anosmia received an ultramicronized supplement containing 700 mg of palmitoylethanolamide (PEA) and 70 mg of luteolin.

PEA (palmitoylethanolamide) is a fatty-acid-derived molecule your own body produces. It’s also found naturally in foods like egg yolk, soybeans, peanuts, and milk.

Luteolin is a plant flavonoid found in celery, parsley, thyme, chamomile, and green peppers.

The supplement was taken as a single dose 5–10 minutes before breakfast. PEA and luteolin are anti-inflammatory/neuroprotective compounds that aim to dampen lingering neuroinflammation in the olfactory system, which is thought to cause persistent smell loss after COVID. PEA supplements must be micronized or ultramicronized for proper absorption and effect.

The olfactory training was structured "sniff therapy" done three times a day. Patients smelled four essences (lemon, rose, eucalyptus, and cloves) for 6 minutes per session. Within each session,, they sniffed one odor for about 4–6 seconds, rested for 40 seconds, then sniffed the next odor, and so on. Patients did this at home after being shown how, with check-ins every 15 days. The training is repeated, structured exposure to the four fixed odors to stimulate and regenerate the olfactory neurons.

The control group did the same olfactory training but instead took a placebo supplement (a multivitamin with low-dose vitamin D and/or alpha-lipoic acid), chosen at doses believed to have no real anti-inflammatory effect.

There were significant differences between the two groups.

In the supplement group, 92% of patients improved their smell, versus only 42% in the placebo group.

The supplement group's average score jumped from about 20 to 30, while the placebo group barely moved (18 to 19.5).

Returning to a fully normal sense of smell happened in 56% of the supplement group but only about 10% of the placebo group.

The improvement built up over time: a hint of benefit appeared by 30 days, became statistically meaningful by 60 days, and was strongest at 90 days.

“A total of 185 patients, including intervention (130) and control (55) were enrolled. The intervention group showed significantly greater improvement in olfactory threshold, discrimination, and identification scores compared to controls. Overall, 92% of patients in the intervention group improved versus 42% of controls. Magnitude of recovery was significantly greater in the intervention group versus control (12.8 vs 3.2), with >10-fold higher prevalence of anosmia in control versus intervention groups at the 90-day endpoint.

Conclusion: Among individuals with olfactory dysfunction post-COVID-19, combining PEA-LUT with olfactory training resulted in greater recovery of smell than olfactory training alone.”
