The question of fluid administration for sepsis in the setting of a patient at risk for volume overload (e.g. CHF, CKD, etc.) comes up almost daily.

In this systematic review with meta-analysis published last week, the authors looked at 37 studies with over 28,000 patients. The studies were a mixture of prospective and retrospective observational data. The studies were looking at a "high" versus "low" volume of fluid administration, which mostly converged around 30mL/kg.

The gist is that they found:
-No difference in 30 day mortality
-No difference in ventilator days, ICU days, or hospital days
-No differences in intubation rates

So the take-home message is that if they need fluids, give them fluids, even if they are at risk for volume overload.

These studies are limited by the observational nature. That said, it is exceedly rare for an RCT to find a difference when no differences were observed with observational data. The differences should be more exaggerated with observational studies.
