@DrDiGiorgio In 2010 our hospital rolled out inpatient EPIC EHR and told us it was start by taking us 50% longer to round on patients and perform all the necessary tasks and when we were good at it, it would only take 20% longer. Per physician productivity instantly dropped never to recover
@hjluks Seems the problem now is the large language models still tell people they have impingement and they come in expecting all the old treatments then look at you like a skeptic when you tell them Chat GPT reads too much old information.
This story is now more than 25 years old and I have told it more times than I can count, but it hits very differently today.
I was anchoring SportsCenter one afternoon and Lou Holtz was on the show. I was quite excited to talk with him, he had been an icon all of my life.
He was very friendly, asking me all about myself as we walked toward the studio to record an interview. I told him: “Actually, Coach, it’s quite exciting, my wife and I are expecting our first child in the next few weeks.”
He stopped dead in his tracks and put a finger up near my face. And I’ll never forget what he said.
“Young man, the most important thing you can do for a child is make sure every day they know how much you love their mother.”
And, just like that, he started walking again.
Our daughter was born a month later, our son came two years after that. And I have thought about what Lou Holtz said to me that day about a million times since.
RIP Coach, thanks for the best advice anyone ever gave me.
Hot take: PE owned firms telling patients it will take a month to get a copy of their X-rays to take to an outside doctor are the worst. Change my mind.
@ArthroToday Outcome may be the same but the cost of care is totally different for patients that can leave the hospital on the same day versus staying over.
@northwoods1980 If positioned well the screw doesn’t cut out, the nail just breaks right where this one did. Weakest point of the nail. They fail in varus just like this. Hence the cpt code for conversion of prior hip surgery to total hip arthroplasty.
@jmills1955@HeartOTXHeartMD As an orthopedic surgeon when a patient gets to prn status I tell them “I hope you never need me again, but send your friends”.
@JJFRAMP@DrDiGiorgio Provision in the ACA prohibits physician ownership of a hospital (some that existed at that time were grandfathered in). No new physician owned / led hospitals since March of 2010 which is around that last time you may have thought your health insurance premium was reasonable.
@northwoods1980@jointdocShields I get a better sense of varus/valgus deformity with the neutral one which is key to some decisions. I get the flexion for patients over 45 because of what you show above. Also some insurances are requiring “bone-on-bone” radiographs for approval of TKA.
@spartywrx I agree with you we typically do not use KL for hips in ortho, apparently the ACR does. Just the usual barriers to care with the insurance company enjoying keeping “the float” and the patient keeping “the pain”

En abril de 2025 Katy Perry quemó 498 toneladas de combustible para hacer turismo espacial. Hoy llegó a Davos a concientizar sobre el "medio ambiente".
@DrDiGiorgio This 💯 if they put half the effort into the front end of the equation they could improve through put dramatically. They also fail to understand is that you can optimize rev cycle all you want but if you are not attracting new patients and retaining current ones you still lose.
