In my experience, the most missed diagnosis in medicine is deconditioning. It’s extremely common and occurs in those who sit a lot, and don’t walk much or otherwise exercise. It presents as fatigue and breathlessness on exertion. Majority of patients referred to me for possible heart failure have this - they are just out of shape.
