The problems.
Time and Cost: to learn the Holep technique you need at least 40 cases, a laser and $150K equipment
Operation time: takes longer than other different ones (Open, RTUP, etc.)

We currently know that there is no spread of prostate cancer in a transrectal biopsy, either to the capsule or the rectum. The probability of it implanting in the bladder in a morcellation should be minimal since it is not by contact, it does not even touch the tissue. Remembering that the spread is lymphatic.

El HoLEP es más "radical" que una terapia focal siempre y cuando la lesión esté en la zona transicional enucleada. De ser así todos estarían tecnicamente curados y el PSA sería a el mejor indicador de cura y/o progresión como ya se ha demostrado
