@Doctors_GUILD Away from the multiple choices; this patient have total collapse of left lower lobe with shifting of the cardia shadow/ sellout to left side and compensatory hyperinflation (Rt side more then 8 ribs), even Lt upper lobe. we cannot role out effusion, but bedside Lung US will help

In echo, the mitral valve seagull sign refers to a distinctive wing-like distortion or "kink" in the middle of the anterior mitral valve leaflet during systole, mimicking the wings of a flying seagull.

Aortic Valve Stenosis Echo Assessment

In suspected AS assess:
1. Vmax and mean PG
2. Aortic Valve Area (AVA) by continuity equation

If there's discordance between mean gradient and AVA >>> Check flow state (SVi) and LVEF

3. SVi, stroke vol­ume in­dexed for body sur­face area to assess Flow state
4. LVEF

■In High flow states with AVA more than 1 cm2 check if it's reversible or not; if reversible >> Reassess under normal flow; if not reversible >>> Severe AS

》High flow may be re­versible (anaemia, hy­per­thy­roidism, or ar­te­ri­ove­nous fis­tu­lae). Up­per lim­it of nor­mal flow us­ing pulsed Doppler: car­diac in­dex 4.3 L/​min/​m2, SVi 58 mL/​m2.

■If low flow states with AVA less or equal 1 cm2>>> Check LVEF

》If LVEF ≥ 50 Cardiac CT to assess Calcium Score

》If LVEF less than 50 either
1. Cardiac CT and / or
2. Dobutamine Stress ECHO to assess Flow Reserve

》Flow re­serve: ≥20% in­crease in stroke vol­ume in re­sponse to low-​dose dobu­tamine or if change in stroke vol­ume 10%–20%, cal­cu­late pro­ject­ed AVA.

■Preserved LVEF with low flow (decreased stroke vol­ume) >>> Check for brady­car­dia or un­con­trolled hy­per­ten­sion, which may lead to pro­longed ejec­tion time and re­duced flow rate.

■Integrated Assessment based on clin­i­cal judge­ment (typ­i­cal symp­toms with­out oth­er ex­pla­na­tion), mor­pho­log­i­cal valve changes, LV hy­per­tro­phy (in ab­sence of co-​ex­is­tent hy­per­ten­sion), and con­sis­tent find­ings us­ing dif­fer­ent modes of as­sess­ment.

So remember four steps in assessment of suspected Severe AS
1. Vmax & mean PG
2. AVA
3. SVi
4. LVEF
