With pCSI, we are able to utilize marrow-sparing technique (essentially treating just the thecal sac) hence, pts tolerate pCSI without any meaningful GI/heme toxicity and can continue systemic tx for extraCND disease

Important caveat: Patient selection is critical. Not for all comers.

The technology enables you to deliver a “better” treatment (sterilize the entire CSF which has disseminated disease) with the equivalent toxicity of IFRT (avoiding bone marrow, N/V, esophagitis, etc).
Congrats @JYangMDPhD et al. on this important study. This marks the 5th completed randomized proton/photon trial and the first to demonstrate an OS benefit. Grateful we are moving the needle for our patients with devastating leptomeningeal disease.
