The jerky give-way weakness is what I'd call 'Fool's weakness' - makes a fool out of the examiner!
Two other causes that I'd add to your list are nutritional (B vitamins...) and POLG mutations (SANDO phenotype).
@calderdalecares You might think it's ok for physician associates to see undifferentiated patients in primary care (I think it's unsafe) but it is not appropriate to equate PAs with GP registrars (doctors) as you have done in this video. PAs are not doctors. Make it clear.
