🫁 ARDS… or something else?

Every intensivist has faced it.

A patient arrives with acute hypoxemic respiratory failure, bilateral infiltrates, severe oxygenation impairment, and fulfills all clinical criteria for ARDS.

But what if it is not ARDS?

A timely review published in Intensive Care Medicine reminds us that approximately 8% of patients meeting ARDS criteria have no identifiable classical ARDS risk factor, and some of these patients harbor highly treatable diseases that require a completely different therapeutic strategy.

The authors propose a practical diagnostic framework for identifying ARDS mimickers, particularly when the presentation includes:

🔹 No clear ARDS risk factor
🔹 Subacute symptom onset
🔹 Extrapulmonary manifestations
🔹 Diffuse alveolar hemorrhage
🔹 Unexpected radiologic patterns

The major categories include:

✅ Immune-mediated diseases
• ANCA-associated vasculitis
• Anti-GBM disease
• Idiopathic inflammatory myopathies (anti-MDA5, antisynthetase syndrome)

✅ Drug-induced lung injury
• Amiodarone
• Chemotherapy
• mTOR inhibitors
• Immune checkpoint inhibitors

✅ Neoplastic pulmonary infiltration

✅ Idiopathic disorders
• Acute eosinophilic pneumonia
• Organizing pneumonia (OP)

One of the most useful concepts from this review is that diagnosis should not rely solely on the chest CT.

Instead, intensivists should combine:

📌 Detailed history and medication review
📌 Extrapulmonary examination (skin, joints, muscles, kidneys)
📌 Autoimmune testing
📌 Bronchoscopy with BAL
📌 Pattern recognition on CT imaging

The article highlights several time-critical diagnoses not to miss:

⚠️ Anti-MDA5 dermatomyositis and antisynthetase syndrome
ICU mortality approaches 50%, and up to 20% of patients require VV-ECMO. Early aggressive immunosuppression is essential.

⚠️ ANCA-associated vasculitis and anti-GBM disease
Prompt recognition of diffuse alveolar hemorrhage and initiation of immunosuppression can be life-saving and kidney-saving.

⚠️ Immune checkpoint inhibitor pneumonitis
A rapidly growing cause of severe respiratory failure in oncology patients that often responds to corticosteroid therapy if recognized early.

The key message is simple:

Not every patient fulfilling ARDS criteria has diffuse alveolar damage.

When classical risk factors are absent, the diagnosis should trigger curiosity rather than closure.

Because sometimes the difference between survival and mortality lies in recognizing the mimicker.

كيف ظهر السوريون فجأة في الرأي العام العربي ككتلة لها وزن ورأي ؟!
متى اصبح لهم صوت !؟

هذا سؤال سمعته من شخص مؤدلج اليوم مستغرب من ظهور رأي عام سوري جديد ، مستنكرا مواقفهم ورأيهم وما أسماه تشبيحهم لحكومتهم ! وسمعته أكثر من مرة خلال الأشهر الماضية.

السوري قضى 14 عام يقاتل من أجل هذه اللحظة ، كي يكون له صوت ووزن

السوري بعد 14 عام اصبح لديه شيء يخاف عليه وهو "الوطن" الذي دفع ثمنه من دمه وعقد من عمر أجياله ، وهذا مالا يفهمه المؤدلج لأن السوري اصبح لديه بوصلة يحدد فيها مساره.

السوري مواقفه تحددها مصالح وطنه وهذا مالا يعقله المؤدلج أيضا

السوري يريد استعادة هويته وثقافته المسلوبة ! بل استعادها فعليا بتضحياته

كيف ظهروا فجأة ؟
الثورة كانت رأس جبل الجليد الذي استغرق 14 عام كي يظهر هذا الجبل على السطح، ظهر الجبل اليوم لذلك هم مصابون بالصدمة !
من هؤلاء متى ظهروا ؟
من هم ؟
ماذا يريدون ؟
لماذا يدافعون عن وطنهم هكذا !؟
لماذا هم خانعون لحكومتهم !؟ خانعون !؟
هذا الشعب انتزع كرامته من المجتمع الدولي والقوى الغربية والشرقية ومن بشار والطائفين والزلازل والجوع والبراميل ، هل من انترع كرامته بهذه الطريقة سيستطيع الشرع أو غيره جعله خانع ومستلب الرأي وشبيح ؟!

السوري في المقابل اصبح يشاهد بشكل يومي شخصيات جديدة لاقضية لها سوى هذا الكائن السوري الذي ظهر فجأة علينا ، من شعراء وعمائم حمراء باسم الدين والقومية والقضية ومحاربة الإمبريالية كل هؤلاء يجمعهم عداء لهذا الجبل الذي ظهر فجأة عليهم !

لقد كسب العرب ، شعب خرج من تحت الأرض بعد أن دفنوه لعقود ..

مالا يعرفونه عن السوري أنه سيؤسس دولة عربية وطنية تعيد التوازن للعرب ، دولة ليست غريبة عن هويتها الوطنية والعربية والإسلامية ، دولة ستكون ملتزمة بقضايا العرب وهمومهم ، فهي أقدر على معرفة هذا الهم .. لاتشغلوا السوري عن بناء وطنه في معارك سخيفة لاقيمة لها

السوري يعرف ماذا يفعل وأين يتجه ، لايجب أن يعطيه أحد نصائح وتنظير
كل ماعليكم فعله دعمه حتى يؤسس كيانه وكل من سيدعمه سيذكره التاريخ العربي الحديث بالخير ..

هناك قيادة تواجه صعوبات غير مسبوقة ودولة منهارة وعقوبات وفصائل وأقليات وهويات متقاتلة ، وخلال أشهر بسيطة زالت وتزول هذه الصعوبات شيئا فشيئا
هذه القيادة لو كانت مؤدلجة من اليوم لما وصلت إلى ماتحقق اليوم ، قيادة براغماتية سياسية وحازمة وذكية ، كفرت بالشعارات وآمنت بالواقعية لذلك هي هنا ، حتى لو كان لها أخطاء فهناك شعب يقوم مسارها وهي تستمتع له جيدا وتفهمه أكثر من غيرها .
