"Real security in the therapy relationship is only discovered when the therapist has failed the patient but unlike the past traumatising object is able to empathically attend and respond to the patients distress. This involves the process of re establishing a lost good object"
Wow
Just read perhaps the greatest article about a doctors’ diagnostic skills
Not in the medical literature, but in an economics journal!
If you think about diagnosis, follow along
It won’t disappoint
@RogueRad
@justin_garson 25 years ago, I argued in my doctoral paper there is nothing wrong with a psychiatric epistemology of mental illness (a view among views). What was wrong was this became a psychiatric ontology of MI (the correct view vs all other views) which research evidence doesn’t support.
@JonathanShedler Maybe, or maybe the DBT resulted in > ability for teens to recognize & admit to these experiences. Typically teens wear a mask of ‘I’m fine’ & it’s possible they became more open. Not all declines in well-being measurement r experiential… as u know, #s r only part of the story.
@TheModernNCPsyA Well that’s odd. That’s exactly what the dude at the power company told me last week that HIS job was all about. Huh. Why r the websites in these industries so complicated.
@ObjctRelations@KemtrupTweets@vardaanbhat@JonathanShedler If u think discussing why your client gave u a “5” from 0-10 on “felt respected, cared for, and really heard today” when u thought u were a vibin’ genius the whole time isn’t a discussion about the rshp & the intersubjective space then as the song sez: I got nuttin’ for u, man.
@ObjctRelations@KemtrupTweets@vardaanbhat@JonathanShedler Convos in tx r spent on many things: chair placement, new skirt, concert seating. These r not therapy? U interpret only literal discssn of rshp? Yikes. Impressn mgmt is a constant factor on some level on both sides. U think this is avoided by not measuring alliance & outcome? 🤦🏻♂️
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler I feel u there. I appreciate the folks here who are protecting and defending depth-oriented therapy. Kleinian analysis massively improved my life. I do existential-analytic-body-centered work. Ppl think measures can’t coexist with depth therapies. I’ve been doing it for 20 yrs.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler These comments don’t seem like you. Not sure what ur core issue is with this. I think u misunderstand the context of how richly the feedback can inform the work just for LOLs. I encourage u to go deeper here. 30-40% of clients drop out of tx. Shrinks r smug.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler Alliance ratings are folded into the transference both in terms of how it’s acted out (thru the scores) and how it’s interpreted collaboratively/intersubjectively. These are not contradictory activities and processes. Instead of snark, maybe explore the ideas.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler Sure, data can be a defense against feeling and resonance, but avoidance of feedback too can be a defense against the idea that one’s intuition about effectiveness in any given case is fallible. Which it is. For all of us. I’d rather have an addl way of knowing.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler Not assumptions. They do tell us so. Often they’re sick of arrogant doctors who know everything. One early study of couples tx in Norway by Duncan et al asked this ‘helpfulness’ question 6mos after termination and the vast majority said being asked the Qs helped the process.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler Well of course. Both. The reason for the formal measures is bc therapists think we have those convos but we have them much less often than we think and when we (not the client) think it needs to happen.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler Will humble u about how easily misattuned we can b, about how we miss a crucial moment (esp at the start) & risk blowing up the tx. It can b done stupidly, concretely, bureaucratically, or w open interest as a simple tool. Its scary to do it the latter way. Clients appreciate it.
@KemtrupTweets@vardaanbhat@ObjctRelations@JonathanShedler Kind of a superficial take on the process. I studied and questioned and argued at workshops for 6 years until Miller or Duncan said ‘just try it.’ So I did. In June it’ll be 20 years since. Almost every session. It’s like proj testing. The scores do matter in aggregate but 1/
@JonathanShedler @HermitinTowerIX Exactly, that’s why it was so funny! I liked my deadly-serious psychoanalysis delivered with an exquisite sense of humor.
