Endoscopic removal of colon tumors started in the 1969, & 50 years later it’s still “just a colonoscopy” from patient’s perspective.
For gastroenterologists, it’s another colon cancer prevented & a reminder of the privilege to be able to do what we do on the daily!
We’re fortunate that the risk of esophageal perforation during balloon dilation of strictures is <1%. The first look at those post-dilation tears can sometimes be terrifying..

Virtual chromoendoscopy has similar accuracy to dye chromo for #GIM, but in patients with a family history of gastric cancer or East Asian ancestry, I combine both. The added yield is only 8%, but extra 8% sure of not missing a precancer lesion is a good deal for me.

Large esophageal tear following dilation with a 13 mm Savary dilator in a long-standing caustic stricture. Fortunately, it was not a transmural perforation. This is one of the reasons why I prefer stenting to dilation.
Hemostatic forceps using soft cos di can be used with jaws closed or open. They are particularly useful for removing clots.
Soft coag lets you pluck the clot while simultaneously sealing what is underneath it—the forceps provide traction while the electrical energy concentrates at the tissue interface rather than simply burning the clot.
(I prefer soft coagulation 50-60W)
