@khaycock2@msiuba I have never been very satisfied with Guyton venous return theory. Some have suggested he confused the dependent and independent variables and RA pressure is a function of CO, not the determinant of It. Volume status also has a role in how sensitive the body is to RA pressure.
@SeeFisch The main management of RV infarct in the modern era is supportive care +/- mechanical circulatory support of the RV. The RV is hard to kill off completely, especially if you can maintain normal RV/LV geometric relationships. You just have to make it though first few weeks.

@IM_Crit_ In patients with hyperdynamic LVs you can have a mid LV obstruction as well as LVOT obstruction resulting in oscillating/ineffective SV . Often seen in patients with small, hypertrophied LVs and hypovolemic states such as sepsis Cutting back on isotropy sometimes helps.

Do you encourage early mobility in acute PE patients? In those with signs of myocardial injury, do you delay ambulation for > 48 hrs or encourage early mobility if feasible? #PERT2018@PERTConsortium@GBarnesMD@jaygirimd@BrentKeeling
“Some people are just really good clotters . . . and don’t resolve their clots.” Dr. William Auger describes that acute RV pressure overload alone is insufficient to cause CTEPH and that vascular remodeling is at the core of the CTEPH transition.
