(3/10) Femoral Vein Doppler is going to be the new KING of congestion assessment.

Let's face it, VEXUS can be time consuming. Femoral vein Doppler is a more scalable, easy, and fast Doppler assessment.

Dr. Denault and Dr. Beaubien-Souligny from Montreal are generating a ton of data on the femoral vein.
1) It is associated with adverse outcomes like AKI (similar to VEXUS)
2) It is associated with COGNITIVE DYSFUNCTION post cardiac surgery.

Definitely an area to keep an eye on!

My biggest reflection from being on in the ICU this week is that people over complicate sepsis.

Find the source. Control the source. Start antibiotics.

Avoid the intellectually stimulating but probably less impactful noise:
Fluids vs. Pressors
Which steroid
Fluid responsiveness
CRP
Biomarkers
etc.

If your patient with sepsis is not improving as expected, 80%+ of the time it is a source control issue.
