"The Impact of Confocal Endomicroscopy Combined with Standard Diagnostics in Evaluating Large Pancreatic Cystic Lesions: An International Interobserver Study"

⏭️ 15 EUS-CLE naive endosonographers participated in a 2 phase diagnostic study !!! 🇮🇳🇮🇹🇺🇸

💡Diagnostic accuracy before and after nCLE review was compared using histopathology as the gold standard

✅ 66 cases
✅ Accuracy increased - distinguishing mucinous vs non-mucinous cysts (90 to 96%)
✅ similar results for sub-type specific cysts as well.
✅ nCLE significantly reduced misclassification, lowering mean unnecessary benign resections from 7.2 to 4.1 (p=0.010) and missed neoplastic diagnoses from 4.4 to 0.7 (p=0.001) per observer, enhancing clinical decision-making

Over the weekend, my ICU has been filling up steadily with patients coming in blood vomiting, altered behaviour, brain and liver failure and severe infections, all of whom relapsed on alcohol during the ongoing Onam festival.

Kerala recorded 8260 million rupees Onam liquor sales.

Patients/family cry that hospital bills are too high and ask for discounts. And beg doctors not to mention alcohol on the admission sheet and discharge summary so that their insurance claims are not rejected.

But magically, every time, they find money to indulge on alcohol.

@docdhir Sharing my thought sir,
Repeat cholangioscopy and breaking the impacted stones with in the basket to smaller pieces. Basket may get disimpacted with this and further removal of basket by cholangioscopy guidance, may be grasping with biopsy forceps or snare.
