A carotid-cavernous fistula (CCF) is an abnormal connection between the carotid arterial system and the cavernous sinus, causing arterial blood to flow directly into the venous sinus.

Classic Clinical Features

🔴 Pulsatile proptosis
👁️ Red, congested eye (chemosis)
👂 Pulsatile bruit over the orbit
👀 Diplopia due to cranial nerve III, IV, or VI palsies (VI nerve is most commonly affected)
⬆️ Raised intraocular pressure and decreased vision (in severe cases)

Triad of symptoms (hemoptysis, chronic cough, night sweats) + 🩻B/L apical consolidation, volume loss, & thick-walled cavitation ➡️ suspected Mycobacterium tuberculosis infection.

Mx: RIPE + pyridoxine (Vitamin B6)

This is a case of chronic suppurative otitis media complicated by Mastoiditis
The mastoiditis progressed into a mastoid abscess and that was the swelling with pus, post auricular.

The management of the condition involves, drainage of the pus, administration of antibiotics.

And after a course of antibiotics, a surgery (mastoidectomy) is planned.

Clinical features

• Painful, deep-seated recurrent nodules or boils
Buttocks, groin, and axillae are common sites
• Post-inflammatory hyperpigmentation, scarring, and sometimes sinus tracts
• Chronic relapsing course

Pathophysiology

HS is a chronic inflammatory disease of the hair follicle, leading to follicular occlusion, rupture, and persistent inflammation.

Differential diagnosis

• Prurigo nodularis (usually intensely itchy rather than painful)
• Chronic buttock folliculitis
• Furunculosis

Management

• Weight reduction and smoking cessation if applicable
• Minimize friction and wear loose clothing
• Gentle cleansing and avoid picking lesions
• Topical clindamycin for mild disease
• Oral antibiotics for more extensive disease
• Intralesional corticosteroids for inflamed nodules
• Biologic therapy or surgical intervention in severe, refractory cases

Clinical importance : Recurrent painful “boils” with scarring in intertriginous areas should always prompt consideration of hidradenitis suppurativa rather than simple folliculitis.

Cottage Loaf Sign:

It is also referred to as the "collar sign" or "hourglass sign".

It is a highly specific indicator of diaphragmatic injury, usually resulting from blunt or penetrating trauma (e.g., traffic accidents).

It occurs as a result of right sided diaphragmatic rupture with partial herniation of the liver through the diaphragmatic defect.

The herniated component is separated by a waist or the diaphragm from the larger intra abdominal component.

This shape is remniescent of a cottage loaf particular shape of bread in which larger and smaller roughly spherical balls are squashed together forming a cottage shape.
