What's wrong with this picture? We received a denial of payment from @CMS for my mom's ambulance ride to the hospital less than 48 hours before her death. They said there was no evidence it was medically necessary--but doesn't her subsequent death suggest medical necessity?

The next time you feel frustrated because a patient "should have come in earlier", remember that medicine is intertwined with social factors. This impacts us as physicians, how we treat our patients, and how our patients lives will change because of this treatment.
