@SbrtSean@blally_md@ACRORadOnc That is why James and I developed the SUGAR trial. Same cohort we used in GU-005 but if Decipher >4 using Regulolix for 1 month with SBRT starting week 2. Opens soon at USF/TGH!
@officialKeef After last seeing the Rolling Stones in CLE 22 years ago it was great to see them back here again. Covid cancelled my last chance to see them 2years ago, but it was great to see Keef and Mick rock and rolling along with the rest of the band. What a show!

@SbrtSean Interesting finding, maybe a secondary analysis of Pace B or on my NRG GU-005 trial in the future would add numbers to evaluate this hypothesis.
@TallAllen@SbrtSean Based on 0815 I’d consider 4 months Orgovyx. If his Decipher was high risk SUGAR trial is offering 1 month Orgovyx with SBRT. I will be offering at USF/Tampa General. If he declined HT 8Gy x5 rather than 36.25 Gy or use a SIB of 40 Gy to bulky disease
Congrats @_ShankarSiva &Team
📢Excellent results FASTRACK II phase II trial.#SAbR effective and safe for primary kidney cancer💪🏻Median Tsize 4.6cm.26Gy/SF(33%) 42Gy/3F(67%).Median FU43m,LC & cancer-specific surv💯% 3-y distant-failureFS 97%.Moving FAST to R phase III trial 🔪⚔️☢️
@SbrtSean The data are encouraging. Less toxicity than LDR/HDR and competed in 28 fractions. ROCR could remove the outdated codes disallowing SBRT reimbursement with an IMRT course. In time newer NRG GU series may show 5 fx SBRT alone works using SIB 5/7.25/8 Gy +/- dominate lesion boost
@DrSpratticus @ParikhSimul @DrAndrewLoblaw@rtdess@UH Sadly it’s fair to sad no randomized trials confirm a Brachy benefit. Maybe RTOG 0815 or 0924 secondary analysis might see a signal?
@DrSpratticus @ParikhSimul @DrAndrewLoblaw@rtdess My data from series @UH Case shows great 10yr bDFS and improved DSS using Brachy/beam or Brachy alone in PSMA bases Capromab SPECT staging for localized disease 99 vs 86% in >240 consecutive pts. UCLA data shows 89% MFS Beam/Brachy vs 66 Beam/HT vs 60 Surg in high grade Gleason.
Despite the debate around #MRgRT its impact on #radonc is undeniable. The inability for sites to continue using #MRIdian still seems like a bad dream. But it will soon become reality and patients w/o question are those who immensely lose. What a complete and utter shame.
