The Girl Who Defeated the World's Deadliest Virus. The Rabies.

In 2004, a fifteen-year-old girl named Jeanna Giese picked up a bat she found at church and sustained a tiny bite on her finger. Weeks later, she developed severe neurological symptoms and was admitted to a hospital in Wisconsin.

The diagnosis was rabies, an ancient disease with a terrifying and historically guaranteed mortality rate once clinical symptoms appear. By the time her infection was caught, it was far too late for the standard post-exposure vaccine to save her life.

🚫 Medicines to Avoid in Heart Failure (HF)🫀

🔺 NSAIDs – may cause fluid and salt retention, worsening HF.
🔺Corticosteroids – promote sodium and fluid retention; if needed, use the lowest dose for the shortest possible duration.
🔺 Beta-blockers – avoid starting in acute or uncontrolled HF; start only when stable.
🔺 Pioglitazone – causes fluid retention and can trigger HF exacerbation.
🔺 Alogliptin – linked to worsening HF in moderate to severe cases.
🔺 Vildagliptin – avoid in severe HF due to possible fluid retention.
🔺 Non-DHP CCBs (Diltiazem, Verapamil) – negative inotropic effect; avoid in HFrEF.
🔺 Flecainide & Dronedarone – can worsen arrhythmias and heart failure.

Erythema gyratum repens (EGR)

Pathogenesis:

- Paraneoplastic dermatosis → most commonly associated with lung carcinoma (also breast, esophageal).

- Caused by tumor-induced immune cross-reactivity → antibodies against tumor antigens cross-react with skin antigens.

Clinical Features:

- Rapidly migrating concentric erythematous bands → “wood-grain” or “rings-on-water” pattern.

- Often with intense pruritus.

- Lesions expand at ~1 cm/day.

- Commonly involves trunk and proximal limbs, sparing face, hands, feet.

- Resolves when underlying malignancy is treated.

Treatment:

- Treat underlying malignancy → skin lesions regress.

- Symptomatic relief: topical corticosteroids, antihistamines for pruritus.

- Monitor for occult malignancy if cancer not yet diagnosed.

FAQs:

Q1: Most common associated cancer? → Lung carcinoma.

Q2: Is it infectious? → No. It is a paraneoplastic immune phenomenon.

Q3: Does it resolve spontaneously? → Only with treatment of the underlying cancer.

Q4: How to differentiate from tinea corporis? → EGR shows concentric, rapidly spreading rings with scaling edges, not central clearing.
