🔺 NSAIDs – may cause fluid and salt retention, worsening HF.
🔺Corticosteroids – promote sodium and fluid retention; if needed, use the lowest dose for the shortest possible duration.
🔺 Beta-blockers – avoid starting in acute or uncontrolled HF; start only when stable.
🔺 Pioglitazone – causes fluid retention and can trigger HF exacerbation.
🔺 Alogliptin – linked to worsening HF in moderate to severe cases.

Erythema gyratum repens (EGR)
Pathogenesis:
- Paraneoplastic dermatosis → most commonly associated with lung carcinoma (also breast, esophageal).
- Caused by tumor-induced immune cross-reactivity → antibodies against tumor antigens cross-react with skin antigens.
Clinical Features:
- Rapidly migrating concentric erythematous bands → “wood-grain” or “rings-on-water” pattern.
- Often with intense pruritus.
- Lesions expand at ~1 cm/day.
- Commonly involves trunk and proximal limbs, sparing face, hands, feet.
- Resolves when underlying malignancy is treated.
Treatment:
- Treat underlying malignancy → skin lesions regress.
- Symptomatic relief: topical corticosteroids, antihistamines for pruritus.
- Monitor for occult malignancy if cancer not yet diagnosed.
