Butterfly is teaching POCUS at a New York Fire Department POCUS workshop. Firefighters are learning how POCUS can provide important answers in emergency situations. #Butterfly#POCUS#FDNY#Emergency@FDNY
We published a study:
Integrating Pre-test Probability and Point-of-Care Ultrasound (POCUS) in the Emergency Department (ED) Diagnosis of Small Bowel Obstruction (SBO) https://t.co/cIE1w2gpfQ
Septic shock of unclear etiology with "multiple splenic and renal infarcts" seen on CT abdomen. Transthoracic echo limited views, but reported as 'normal'. Blood culture negative. Resus TEE done showing the following. What's going on? #pocus#resusTEE (see next for poll)
A patient presents with sepsis with a hx of IVDU. No murmur, SOB, or CP. What’s going on here in this RV inflow view?

It’s Friday again - means Quiz time❓❗️Ultrasound shows an unclear round liver mass in a steatotic liver. Next step should always be contrast enhanced ultrasound. What do you see and what is your diagnosis?

RESOLUTION:
S: Convex transducer placed sagittally over the right upper quadrant viewing the liver & gallbladder.
R: Pneumoperitoneum & complex ascites suggests gastrointestinal perforation.
Singular lymph node metastasis of a head and neck cancer. Typical ultrasound features of an inhomogenous lymph node, more round shape than wide and partly anechoic, which represents a necrosis. These metastatic lymph nodes do not hurt❗️

Quick Case.
Patient with a history of recent trauma. He was referred for an ankle ultrasound (normal).
Clearly, the problem is at the mid-foot.
Lisfranc injury, maneuvers are performed with stress, axial compression on the metatarsal with a fixed midfoot, and we can clearly see the marked instability of the medial column, with opening and displacement.
It is much more evident when compared to the normal side.

What's the diagnosis? How much would you drain with your initial drainage (assuming they are hypotensive)? (see next for poll) #pocus#meded#medtwitter#Cardiotwitter
How I use #POCUS for flank pain/ renal colic / urosepsis - yes, I look for hydronephrosis that might triage additional imaging, but more importantly, to look for the company it keeps! Always remember to check out the aorta if you are scanning for a stone.
