@txsportsdoc I would want full length tibia and knee films (examine physes) as well as CT ankle (looking for intra-articular extension given how distal). But definitely considering a nail if I think I can get three interlocks distal to the fracture site.

@InvictaOrtho I like this for the “implant lateral” but at our institution we also get a slightly externally rotated view - a “reduction lateral” to visualize the anteromedial cortical buttress for evaluating the reduction.

@cj_footeMD @traumaticum Exactly. Perc incisions and work through the fracture lines or through a corticotomy to tamp up the joint surface. Raft in place with headless compression screws. Drop down the nail and place fixed angle interlock and then stack screws between interlock and rafting screws.

@traumaticum We usually “haystack” screws until able to rest on a fixed angle interlock through the nail. Screws act like - rebar and the whole thing hangs on the nail. Can all be done percutaneously without having to make bilateral incisions for plates in these bicondylar plateaus.

@InvictaOrtho For my tough closures I prefer using multiple Allgöwer-Donati sutures, thrown loose, then snapped to a hemostat and tightened sequentially.

@HoustonISD Lol sending out an email to us parents with <24 hours notice is neither helpful nor celebratory. Those parents that have the ability to take the time off and bring kids to the parade would done so anyways. Cancelling school just punishes those who rely on the childcare.

@aqueipot I would argue for a slightly lateral of center endpoint for your guidewire/nail. Center endpoints as you describe can translate the distal fragment laterally and lead to valgus with fracture gapping medially. https://t.co/S2nZuFTs46
