Don’t open Codex or Claude Code from your Downloads folder.
Forever Security found an AGENTS.md sitting in a client's ~/Downloads. Looked like normal project notes. It told the agent to edit ~/.codex/config.toml and npm install malware. Codex went along more than 90% of the time in their tests. Claude Code with a planted CLAUDE.md leaked OAuth tokens about half the time.
There’s no patch for this. The agent is just doing its job reading instructions from wherever you opened it. And the browser can drop files into Downloads without you clicking anything.
So yeah, open agents inside the repo, not Downloads. If you see an AGENTS.md or CLAUDE.md you didn’t write, read it first. And glance at ~/.codex/config.toml once in a while for MCP servers or weird base URLs you didn’t put there
Lots of conflicting php2shell advice tonight, so I put what's circulating into one repo, ranked by how confirmed each mitigation is. It has configs for Apache, Nginx, LiteSpeed and Docker, plus scripts to check exposure and look for webshells.
There's no official patch yet, so treat it as a stopgap

If you’d rather not deal with this yourself, I can patch and harden your server, check it for webshells, or help you move off an aging PHP stack. DMs open
