Engineering note: why coins went missing, and the fix.

Two launches landed seconds apart. Both read the same shared list, both wrote it back. Last write won, and one coin silently dropped off /coins.

The fix: every coin now owns its own key (coin:<mint>). The list is a prefix scan over those keys. Two launches can never overwrite each other again.

Old records are still read and merged, so nothing launched before the fix is lost.
