Seriously, only 1 gland of 12 cores - that’s on my second read when i was about to sign it out. Asked for a recut and CA was present in the deeper levels - G4+3. Here is where i leave a comment that tiny tumor is at one end of the core and best seen in level __. #gupath
Prostate bx with - dark cribriform glands. In better sections, you will find glands lined by cigar-shaped cells, papillary architecture with fibrovascular cores seen. DX: Ductal prostatic adenoCA. No basal cells with the PIN4 but may have patchy retention in some ducts #Gupath
Monday, monday! Bladder biopsy with malakoplakia - the pathognomonic finding of MG (Michaelis Gutman) bodies were easily found on H&E and confirmed by the von kossa stain.
When you ordered PIN4 on a separate focus of atypical glands and you just assumed this other focus was high-grade PIN and it turned out to be PIN-like carcinoma.😳 #GUpath 🔬
pt2 or pt3? Added a section of testis at the terminal end of the sc. #seminoma involved the hilum, epididymis and coiled ductus deferens. Involvement of #ductus deferens & #cremastericmuscle - which surrounds the sc, supports pt3 designation. 🔬 🔍 👩🏻⚕️ #gupath
Dark poorly formed glands looking like higrade prostate CA?. PANck was (-). These are histiocytes. On HPO you’ll see the dusky blue-gray cytoplasm - that’s a good clue. PIN4 can be misleading as racemase can leave a pink blush and it will be (-) for p63/CK5/14. Do CD68 #GUPath🔬
Small crowded dark glands which appear to have an infiltrative growth? Look closely, and do PIN4. The basal cells light up with p63,CK5/14. This is #basalcellhyperplasia a benign histologic mimic of #prostaticCA I’ve seen this in fusion bx with a high MRI PI-RADs score. #gupath🔬

Careful…this is not #epe extraprostatic extension. I interpret this as adipose tissue superimposed on the core with tumor. Few other cores have low volume Gleason 6. #gupathology 🔬#prostatecancer#gupath
tattoo pigments in a fragment of colorectal mucosa accompanying a prostate biopsy? Found out surgeons use colonic endoscopic tattooing for 4 reasons-one of these is tattooing near resected polyp that needs surveillance. On follow up, scar will be easier to find #GUpathology 🔬
@daniel_berney I always do one cd117 to one block with seminoma close to GCNis. The one time it was negative, did SF1 - it turned out to be positive - sertoli cell tumor😳
