17 cases w true OMI (TIMI 0/1 flow) were falsely canceled by the interventionist.

Two key limitations:
1/ No “call-offs” included, despite being major workflow disruptions. AI has been shown to reduce ~90% of these.
2/ Did not include the index ECG, just the “diagnostic ECG”, often the last ECG after symptom-driven activation.
Missed STEMIs (NSTEMI-OMIs) have ~7.8% in-hospital mortality.
That’s higher than than the 1-year mortality after STEMI managed in time.
Time is muscle, regardless of ECG criteria. We now have tools to detect a significant proportion of these earlier at acceptable PPV!
