My birthday was February 07. February 09 my daughter rushed me to ER with severe abdominal pain and vomiting. They sent me ho.e with that old tired refrain... "it's just your fibro and Gastroparesis." February 11th I came back in an ambulance. Been here ever since.

@AssessorCook and finally; 3) Do I have to submit copies of supporting documents for each exemption being applied for AND for each year (thus 10 copies of each document), or can I submit one copy of each document in one application that would cover all exemptions and years.
