Deep respect to @LondonProstate1 Prof. Hashim Ahmed for his unwavering commitment to advancing prostate cancer care. Designing and executing RCTs comparing focal ablation to whole-gland radical treatment is notoriously difficult due to patient preferences and recruitment hurdles. Yet, he remains one of the few investigators in the world actually leading multiple prospective RCTs in this space allowing us to build the evidence base we all rely upon.
Clearly you did not read the manuscript and have minimal grasp of radiation biology. They used low dose SBRT, which did have improved QOL/reduced toxicity in select domains, but will not show superiority with those doses which are not standard today.
Most people who use that Rx give a 120% hotspot, but they did not even allow dose much over 38 Gy to hotspots!
PACE-B gave Rx of 40 Gy with hotspots over 44 Gy. This may not seem like a lot to you, but it is a big difference.
Hilarious this is coming from the biggest proponent of focal therapy who cant even run a single RCT to prove it helps patients.
We applaud @LondonProstate1 Prof. Hashim Ahmed's longstanding commitment to improve the care of our patients with prostate cancer, specifically through his visionary leadership of multiple RCTs in the pioneering field of focal therapy. It is not easy to compare partial treatment with standard of care (recruitment issues etc), yet he has shown the tenacity to plan and execute these monumental efforts helping to lead the way for us all.
