Senator @TomClonan made serious allegations in the Seanad concerning child protection practices within HSE West and North-West (covering Sligo, Leitrim, Roscommon, and Donegal). He claimed that management bullied, intimidated, and obstructed clinicians who attempted to report suspected child sexual abuse.

The Government confirmed an independent external review is underway but declined to comment on individual cases.

Senator Clonan described the situation as one of the gravest matters he has encountered. Three HSE clinicians approached him, believing they were blocked from fulfilling their legal duties under the Children First Act. He stated these "people of conscience" faced workplace reprisals for trying to report concerns.

In one instance, a clinician allegedly discovered a child may have been sexually assaulted or raped. Despite a legal obligation to report this to Tusla and An Garda Síochána, Senator Clonan alleged a senior manager instructed them not to.

He noted that failing to make a mandatory report, or directing clinicians not to report, constitutes criminal wrongdoing, yet no disciplinary action was taken against the manager.

A second incident involved a woman at a counselling clinic disclosing that her husband was accessing child sexual abuse material, raising immediate risks for local children. When clinicians tried to report this, they reportedly faced resistance from the same management team, with one clinician allegedly accused of having a mental disorder.

Senator Clonan described email correspondence between the clinicians and management as “appalling.” He added that formal protected disclosures made in February 2025 had still not been properly processed 18 months later, questioning whether families in the region could have confidence in local child protection.

Drawing comparisons to past abuse scandals, he warned against repeating historical mistakes.

Responding for the Minister for Health, Minister of State Frankie Feighan acknowledged the seriousness of the matter. He confirmed that HSE West and North-West is conducting an independent external review under the HSE Incident Management Framework into a north-west counselling service.

Two stages of the review are complete, and while complex documentation has caused delays, any final recommendations will be fully implemented.

Minister Feighan reiterated that under the Children First Act 2015, mandated professionals have a personal legal duty to report child protection concerns to Tusla, which cannot be delegated.

He noted that following a letter to the HSE Chief Executive in October 2024, the CEO explicitly instructed the region that line managers must not screen or block mandatory reports.

While staff are encouraged to consult managers, this must never become a barrier. If a disagreement over thresholds occurs, staff are free to report directly to Tusla or the Gardaí. Feighan concluded by encouraging anyone requiring mental health or counselling services to continue seeking HSE support.
