1/ Anaesthetic challenge.
Patient turns up at a district general hospital for urgent (cancer) surgery. 2 months earlier TTE showed this pericardial effusion considered to be neoplastic in origin. Colchicine and steroids had been prescribed. A repeat echo the day before surgery was missed. You look at the scan 👀🫀”
The GMC wanted to redact who they were seeking support off to influence the Leng review
Unfortunately they didn't redact it all
"Potential KF support"
Now what organisation would have the resources for such a task, the appearance of independence, and have the letters KF…

Gigi is the most beautiful, precious little girl. Her death was entirely preventable, due to unsafe sleeping arrangements at nursery. Her wonderful Mum & friend of mine, Katie, is campaigning for safer nursery environments. Please share #campaignforgigi

Totally outrageous attitude in this day and age. Sad that ICM has lost someone talented.
ICM consultant with two children here, happy to chat to anyone who might have come across these attitudes and be having second thoughts about perusing a career.
🔥Have been incensed about something all day….
Women are the majority of medical students in the UK, yet there are still pockets of the medical profession where dinosaurs are alive, well & roam the earth.
Turns out my own clinical specialty (ICM) is sadly one such place….
1/8
PAs being enabled to become senior clinical decision makers after their (2-6 week) placement as a student
For comparison, Drs become Snr clinical decision makers after:
✅ 4-6yrs med school
✅ 2yrs foundation training
✅ 4yrs specialty training
✅ RCEM specialist exams
I have honestly no idea what it would take to make it up to candidates affected by this scandal (and it is a scandal).
Paying for a resit isn’t enough.
What about the candidates who were mistakenly told they had failed, who then may have left training?
Awful
I agree, I am fortunate enough to have breastfeed both babies, but not without additional formula in the early days because as you say, breast feeding is hard and both babies became jaundiced and would have been admitted without us giving formula. The formula was our intervention
'The watchdog said parents often chose... formula...when they were vulnerable, often in hospital immediately after birth, when they did not have “clear, accurate and impartial information needed to make informed decisions”'
Let's unpack this, shall we!🧵

This chart is a scandal.
Teachers, doctors and others earning fairly ordinary salaries can face marginal tax rates of more than 60%, and sometimes approaching 80%.
Rachel Reeves should reform income tax. Thread:
📣 Only few places left!
🥼Clarification: The event is open to any trainee interested in ICM - core/foundation/LED etc - use the FICM trainee rate in person. Also any MDT can attend online for the lecture portion.
📝 Abstract submission will be extended till 18th September
Giving a human factors talk next month at a critical care conference
Hmm... BMJ study found ICU trainees urine was x2 as concentrated as their patients
Even a 1-2Kg loss of body water can reduce analysis/decision making by 15-20%
Drinking regularly helps maintain performance
@doctimcook@stephen_t_webb@RCoANews Thank you @RCoANews it is good to have the college encouraging the use of professional titles. Since becoming a consultant I have had to ask to have Dr put on my ID badge and have the name board of consultants updated as I had been ‘un-doctored’.
Bravo to everyone working on the critical shortage of IV fluids in Australia.
Once we get through this acute problem we need to seriously ask ourselves some tough questions.
Let's start with - why on earth do we buy fluid 14,000 km away from Norway?
@GPswampwarrior My relative was told had shingles by a pharmacist, he had a rash that was non dermatomal, crossed the midline and was itchy not painful 🤷♀️
I am really so sick of the extent to which the HCPC
@The_HCPC does not care EVEN REMOTELY about those it regulates.
I have a #FitnessToPractise case going on since September 2020 - nearly 4 years hanging over the registrant & putting them at significantly increased risk of death by suicide.
Got invited to a short hearing on the case this week. I say "Is this an error, I'm not aware of any upcoming hearing?".
Response - "Oh the HCPC decided to discontinue the case against your client, it's just that nobody bothered to tell him. Or you. The hearing is for the HCPC's discontinuance application".
There are multiple stages to deciding to discontinue, making that application & arranging a hearing. The registrant is of so little importance to the HCPC that nobody, at any point in the process, thought to inform the registrant himself that his ordeal would be coming to an end.
This says EVERYTHING about the HCPC's toxic attitude to registrants, which runs right through the FtP process.
