Why does a proton beam stop where a photon beam continues?

The answer is the Bragg peak. Protons deposit their maximum energy at a controllable depth, followed by a rapid distal fall-off. By combining several energies, a spread-out Bragg peak can cover the complete tumour while reducing radiation beyond it.

This makes proton therapy particularly valuable in children, craniospinal irradiation, ocular melanoma, skull-base chordoma, tumours beside critical organs and selected re-irradiation cases.

But dosimetric elegance does not mean universal clinical superiority. Proton range changes with tissue density, motion and anatomy, while biological effectiveness may increase at the distal edge. Robust planning and careful patient selection are therefore essential.

Resultados interesantes, sí. Milagros, no. ⚠️🔬

Un reciente metaanálisis de ensayos clínicos ha analizado la suplementación con #Creatina en PACIENTES con #Cáncer. 🎗️🧪

📊 El trabajo reunió 5 ensayos aleatorizados con 373 pacientes y encontró que, frente al placebo:

🚫 💪 No hubo mejoras significativas en fuerza muscular.
🚫 🦵 No hubo mejoras significativas en masa magra.
🚫 🚶 No se observaron beneficios claros en función física.
🚫 ❤️ Tampoco mejoró significativamente la calidad de vida.
✅ 📉 Sí apareció una reducción media de unos 2,8 kg de grasa corporal, pero la certeza de esta evidencia es baja y el resultado debe interpretarse con cautela.
✅ 🛡️ No se detectó un aumento claro de eventos adversos, aunque los datos de seguridad disponibles todavía son limitados.
🔹 🧫 La información específica sobre función renal procede de muy pocos estudios, por lo que tampoco conviene hacer afirmaciones absolutas.

🚨 Y esto es especialmente importante: estos ensayos NO demuestran que la creatina combata el cáncer, reduzca los tumores ni aumente la supervivencia.

🐭 Otros trabajos recientes han encontrado mecanismos inmunológicos antitumorales interesantes en células y modelos animales. Son investigaciones prometedoras 🔬✨, pero todavía existe una enorme distancia entre conseguir un efecto en un ratón y demostrar que un suplemento mejora el pronóstico de un paciente oncológico.

⚠️ En oncología debemos ser especialmente cuidadosos con los mensajes que lanzamos. Una hipótesis prometedora puede justificar nuevos ensayos clínicos; no justifica venderla como un beneficio demostrado.

🎗️ Las personas con cáncer no necesitan que les vendamos ilusiones. Necesitan información rigurosa, distinguir claramente entre resultados preclínicos 🐁, evidencia clínica 👩‍⚕️ y tratamientos realmente demostrados ✅, y conocer también las incertidumbres y los posibles riesgos.

🔬 La creatina es uno de los suplementos mejor estudiados en otros contextos, especialmente en ejercicio y rendimiento muscular. Pero eso no convierte automáticamente cada nuevo resultado experimental en otra razón para que “todo el mundo tome creatina”.

📚 Prometer menos y demostrar más también es hacer buena ciencia.

Why Add Celecoxib to Immunotherapy?

The PICC-2 trial explored whether blocking the COX-2/PGE₂ pathway could enhance the activity of PD-1 blockade in dMMR/MSI-H locally advanced colorectal cancer.

By reducing immune suppression within the tumor microenvironment, celecoxib may help T cells eliminate residual cancer more effectively.

Pathological complete response improved from 69% to 89% with the addition of celecoxib.

The combination achieved this benefit without increasing treatment-related toxicity.

A promising strategy that warrants further validation, but adds an intriguing layer to neoadjuvant immunotherapy.
